If you are a board member or
general member who has incurred personal expenses on behalf of DCBA, and you
need to request reimbursement of your expenses, please use this form.
DCBA Expense
Reimbursement Form. You will need Adobe Acrobat Reader on your computer.
Most computers have this program installed by default. If yours does not you
may install it at no cost by clicking the "Get Acrobat Reader" icon below.
You may fill this form out
within Acrobat and submit it using the Email button. This will
open your email program and attach the form to the email. Please email to Larry Johnson.
l.johnson@derbycitybop.com . You may wish to save the form to your My
Documents folder naming it anything you will recognize. Open your email
program and send an email to Larry Johnson using the email address above.
Attach the filled out and saved Acrobat file to the email. You will need to submit receipts
substantiating your request for funds. If you have a scanner and understand
how to scan the receipts to a tiff or jpeg or pdf file, please feel free to
do so. Simply attach these images to the email you are using to submit the
expense request form. Larry will receive the form and receipt copies via
email and prepare your reimbursement.
If you do not have a scanner or
if you do not understand how to scan your receipts to one of these image
formats, no problem. You have two choices. Open the form in Acrobat and fill
out the fields within Acrobat. Print the form with the fields filled in.
Give the form and your receipt copies to Larry Johnson for reimbursement. If
you wish to speed up this process a bit simply fill out the form and email
it to Larry. He will prepare your reimbursement. You will need to give Larry
copies of your receipts substantiating your request for reimbursement at the
time he provides your check.
Board members who might wish to
carry copies of this form may print the blank form. You may fill the form
out longhand and give the form and your receipt copies to Larry for
reimbursement.
